How to report a product complaint?

For ROOTT Dental Implants, Abutments, Caps, Clinical Screws and Surgical Instruments.

We, TRATE AG, take all product complaints very seriously. Our main goal is to identify the root cause of product failures, implement corrective and preventive measures and include these outcomes into the new product developments. In order to implement this and to comply with regulatory requirements for the market surveillance, we kindly ask you to complete our product complaint form.

How to submit a product complaint?

  1. Please, fill the online complaint form and press “Submit”.
  2. Save the confirmation email you receive.
  3. Decontaminate the product which is the subject of the complaint in accordance with our Instruction for product return
  4. Prepare copies of supporting documents as required by the Warranty and return policy
  5. Pack the product with the printed complaint form and copies of supporting documents and ship it to the following address of the Manufacturer:

Upon receipt, we will review your complaint and provide a replacement product according to our Warranty and Return Policy.
